Ruffled fur/hair in the back of neck? Empty Ruffled fur/hair in the back of neck?

Hi! I'm fairly new, what does it mean when the hair on the back of their neck looked ruffled/unkepted/oily or like I can see the epidermis a little? What should I do? Lessen the protein, and reduce the stress 0’;
